WHAT COVERAGE DO I HAVE AND WHAT STEPS DO I TAKE IF INJURED?

Your BMMSL Membership also provides you with secondary sport accident insurance. “Secondary” means that you must first use any insurance benefits already available to you through the Ontario Health Insurance Plan, your employment, a partner, spouse or parent (your “Primary” insurance), before this coverage can be used.

Some reimbursements have limits and/or caps, and it is not a loss of wage or income policy, but it is the most comprehensive sport accident insurance available to players.


Player safety is our top priority.
If you are injured during a league game or practice, please follow these steps to ensure proper care and access to Supplementary Insurance Coverage:

  1. Report the Injury Immediately (INCIDENT REPORT FORM)
    Notify your captain or a League Executive as soon as the injury occurs - on the field or shortly after. Timely reporting is crucial.  You have only 48 hours to submit an incident report to SPO.  You can download the fillable form here. 
    ** You will require league/umpire/insurance ID# details on the form - which the League President will provide you.  When the form is complete - email it to kaylee@playslopitch.com.

  2. Seek Medical Attention - If Required
    Visit a doctor, walk-in clinic, or hospital as needed. Keep all ORIGINAL medical records, receipts, and documentation related to the injury.  Pictures of receipts will not be acceptable.

  3. Report the Claim - If Required (SPORT ACCIDENT FORM) 
    You must advise SPO within 30 days of any injury claim.  Within 45 days, you will need to complete a  Sport Accident Report Form for an official claim. You must email kaylee@playslopitch.com to request a form directly.  Once completed, the form plus all medical receipts for claim support must be submitted in full. 

  4. Claim Processing
    SPO will process your official claim and forward all documentation to the insurance company.  The insurance company will contact you should they require any further details.  You will have up to 52 weeks to submit any further details required.  Once a claim is approved, you will be notified.

  5. Stay in Touch
    If you require follow-up care or time off, please keep your captain and League President updated.  We will support you as well as your team with a spare player if necessary.
